1. Schedule your first appointment
Please select “Direct Primary Care – Initial” for your appointment type for your first appointment. Your first appointment must be held in person. Please allow for at least one week between now and your appointment request to fill out paperwork.
2. Complete your forms in your Tebra Patient Portal
After scheduling your appointment, keep an eye out for your intake forms (sent within 1 business day of appointment request). These forms must be completed before we approve your appointment request You’ll complete these in your Tebra patient portal. Use your Tebra patient portal for communicating with your provider, scheduling future appointments, and accessing the telehealth platform.

Amanda is a board-certified Family Nurse Practitioner and military spouse with six years of clinical experience caring for patients across the lifespan. She has trained in internal medicine, obstetrics, postpartum care, and pediatrics, giving her a well-rounded foundation in both preventive and complex care. Her passion for women’s health and patient advocacy drives her commitment to meaningful continuity of care at every stage of life.

You’ll receive the same thoughtful, relationship-based care you’re used to, including 45-minute visits (not the industry-standard 7-minute rush). We continue to bill insurance for bioidentical hormone therapy and GLP-1 medications when eligible. More access, same high standard of care — schedule now while availability is open.

Patient and Membership Portals
All Thrive patients have a patient portal. In this portal, you can message your care team, check labs and documents, and share files.
